Use an order-specific questionnaire link when a patient needs to complete an intake/questionnaire for a specific order, including forms required before an order can be submitted to Beluga.

What To Replace

• [form_id]: The questionnaire/form UUID the patient must complete.
• [order_id]: The internal HLC order UUID related to the questionnaire.

Do not use the visible order number if it is different from the internal order UUID.

When To Use This

Send this link when:

• A patient has a specific order that is waiting on an intake/questionnaire.
• The order cannot be submitted to Beluga until the required questionnaire is completed.
• CS needs the patient to complete the questionnaire in the context of that exact order.

Important Notes

• The patient must sign in to their patient account to access the form.
• The order must belong to that patient.
• The form must be assigned/mapped to that order.
• If any of those checks fail, the patient may see an access or assignment error.
• Keep the ?orderId= part in the URL. Without it, the form may not be connected to the order.
